Acute urinary retention (AUR) is an extremely uncomfortable and potentially life-threatening condition characterized by a sudden inability to urinate associated with intense suprapubic discomfort. An Analysis of the Social Gradient of Health in Relation to the Australian Indigenous population â€Å"The demonstration of a social gradient of health predicts that reducing inequality itself has health benefits for all, not simply for the impoverished or deprived minorities within populations. † (Devitt, Hall & Tsey 2001) The above quote from Devitt, Hall and Tsey’s paper is a relatively well grounded and well researched statement which draws on contemporary theoretical sociological concepts to support the assertion that reducing inequality is the key to improving health for all. However the assertion that the demonstration of a social gradient of health predicts that a reduction in inequality will lead to health benefits for all is a rather broad statement and requires closer examination. The intention of this essay is to examine the social gradient of health, whose existence has been well established by the Whitehall Studies (Marmot 1991), and, by focusing on those groups at the lower end of the social gradient, determine whether initiatives to address inequalities between social classes will lead to health benefits for those classes at the lower end of the social scale. The effectiveness of past initiatives to address these social and health inequalities will be examined and recommendations made as to how these initiatives might be more effective. The social gradient described by Marmot and others is interrelated with a variety of environmental, sociopolitical and socioeconomic factors which have been identified as key determinants of health. These determinants interact with each other at a very complex level to impact directly and indirectly on the health status of individuals and groups at all levels of society; â€Å"Poor social and economic circumstances affect health throughout life. People further down the social ladder usually run at least twice the risk of serious illness and premature death of those near the top. Between the top and bottom health standards show a continual social gradient. † (Wilkinson & Marmot 1998) In Australian society it is readily apparent that the lower social classes are at greater disadvantage than those in the upper echelons of society; this has been discussed at length in several separate papers on the social gradient of health and its effects on disadvantaged Australian groups (Devitt, Hall & Tsey 2001, Robinson 2002, Caldwell & Caldwell 1995). Within the context of the social gradient of health it can be inferred that Indigenous groups, for example, are particularly susceptible to ill health and poor health outcomes as they suffer inordinately from the negative effects of the key determinants of health. A simple example of this is the inequality in distribution of economic resources: â€Å"Average Indigenous household income is 38% less than that of non-Indigenous households. † (AHREOC 2004). The stress and anxiety caused by insufficient economic resources leads to increased risk of depression, hypertension and heart disease (Brunner 1997 cited in Henry 2001). Higher social status and greater access to economic resources is concomitant with a reduction in stress and anxiety levels, as individuals in these groups have more control over economic pressures which create this stress. This simple comparison proves that the social gradient of health accurately reflects how socioeconomic determinants affect the health of specific social classes at the physiological level. An extension of the research into the social gradient and the determinants of health is the examination of the pathways through which specific social groups experience and respond to these determinants. These ‘psychosocial pathways’ incorporate psychological, behavioural and environmental constraints and are closely linked to the determinants of health; â€Å"Many of the socio-economic determinants of health have their effects through psychosocial pathways. † (Wilkinson 2001 cited in Robinson 2002). These pathways have been demonstrated by Henry (2001) in the conceptual model of resource influences (Appendix A), a model which illustrates the interaction between the constraints mentioned above and their impact on health outcomes. Henry states that a central differentiator between classes is the amount of control an individual feels they have over their environment. Whereas an individual from a lower class group holds a limited sense of control over their well being and consequently adopts a fatalistic approach to health, those in higher classes with a stronger sense of control over their health are more likely to take proactive steps in ensuring their future wellbeing. This means that both individuals will cope differently with the same health problem. This is partly as a result of socioeconomic or environmental determinants relative to their situation, but it is also a result of behavioural/physical constraints and, most importantly, the modes of thought employed in rationalising their situation and actions. The children on the island, particularly Jack’s tribe, have made many poor choices out of fear. The boys were getting wild and rowdy doing their rain dance to get rid of the rain because they were afraid of the thunder and lightning. Instead of exploring the island and finding out what they were afraid of , Ralph and Jack were busy trying to decide who would be the leader of the island. Fear of the beast got the boys scared that he beast was going to come down and kill them, Simon thought other wise. On the island fear of each other caused another death. Fear caused division amongst the boys which led to another death on the island. Ralph, Piggy, and Sam and Eric were going to get Piggy’s specs back. Piggy expressed his concern Rivera 2 about going because he was afraid of Jack. He had reason to be afraid of Jack because he was bigger, stronger and had intimidated him in the past, for instance, he would often tell Piggy to â€Å"Shut Up! (Golding 37). Jack was afraid that Ralph would be the leader instead of him. When Ralph and his crew showed up at Castle Rock they noticed that Jack’s tribe had their faces painted black like savages. Jack ordered Roger to get the boulder trap to get rid of Ralph and his crew. Piggy and Ralph put their fear aside and went up to tell Jack that the glasses were Piggy’s. Meanwhile, Roger, out of fear that Jack wou ld hurt him if he didn’t obey his orders, let the boulder trap go. The boulder hits Piggy and he falls off the cliff and dies. Jack covers up his fear by saying that he had planned it all along. As you can see, fear often leads people to make wrong choices, and unfortunately they are left with the consequences of their actions, or the lack of their actions depending on how they let fear manipulate them. The saying its better to be safe then sorry is a great way to sum up how people let fear control them. Being fearful is not always a bad thing, but when you let it completely take over how you react to every situation it becomes a bad thing. For example, if your friends are doing something you know is wrong but you’re scared they wont like you if you don’t do it, and you go ahead and do it any way, that is being a coward. People who run from the police because they are scared to go to jail, is a bad decision many people often make out of fear. People in the world often let other people’s fear control them. When at an amusement park, deciding not to go on a ride because a friend is scared, would change the outcome of the trip from having a good time to being bored. Often in politics people all vote the same way because someone else is telling them to and they are fearful of what people will think of them if they vote against crowd. There are many things people fear, the unknown, finances, love, happiness, death, just to name a few. Knowing when to let go of some of the fear is the important part. Being confident in life Rivera 3 and sticking up for what one believes in helps people become confident and not afraid all the time. Learning the difference between being cautious and being afraid is what will people in society must learn to not live in fear of living. In Lord of the Flies there are two perfect examples of how fear controlled the minds of the kids which eventually led to the deaths of Simon and Piggy. Fear in the world can cause things to go wrong, because it makes people hate each other and be prejudice. A lot of bad decisions are made out of fear that hurt others. Learning to live without fear is something that people should try to learn to do. First Indochina War In September 1945, Ho Chi Minh proclaimed the Democratic Republic of Vietnam and named Giap as his interior minister. The government was short-lived, however, as the French soon returned to take control of the area. Since the French were unwilling to recognize Ho Chi Minhs government, fighting soon broke out between the French and the Viet Minh. Given command of the Viet Minhs military, Giap soon found that his men could not defeat the better-equipped French and he ordered a withdrawal to bases in the countryside. With the victory of Mao Zedongs communist forces in China, Giaps situation improved, as he gained a new base for training his men. Over the next seven years, Giaps Viet Minh forces successfully drove the French from most of North Vietnams rural areas; however, they were unable to take control of any of the regions towns or cities. At a stalemate, Giap began attacking into Laos, hoping to draw the French into battle on the Viet Minhs terms. With French public opinion swinging against the war, the commander in Indochina, General Henri Navarre, sought a quick victory. To accomplish this he fortified Dien Bien Phu, which was situated along the Viet Minhs supply lines to Laos. It was Navarres goal to draw Giap into a conventional battle where he could be crushed. To deal with the new threat, Giap concentrated all of his forces around Dien Bien Phu and surrounded the French base. On March 13, 1954, his men opened fire with newly obtained Chinese guns. Surprising the French with artillery fire, the Viet Minh slowly tightened the noose around the isolated French garrison. Over the next 56 days, Giaps troops captured one French position at a  time until the defenders were compelled to surrender. The victory at Dien Bien Phu effectively ended the First Indochina War. In the ensuing peace accords, the country was partitioned, and Ho Chi Minh became the leader of communist North Vietnam. Vietnam War In the new government, Giap served as the minister of defense and commander-in-chief of the Peoples Army of Vietnam. With the outbreak of hostilities with South Vietnam, and later the United States, Giap led North Vietnams strategy and command. In 1967, Giap helped oversee the planning for the massive Tet Offensive. Giap was initially opposed to a conventional attack; he had goals that were both military and political. In addition to achieving a military victory, Giap hoped the offensive would spark an uprising in South Vietnam and show that American claims about the wars progress were wrong. While the 1968 Tet Offensive proved to be a military disaster for North Vietnam, Giap was able to achieve some of his political objectives. The offensive showed that North Vietnam was far from being defeated and significantly contributed to changing American perceptions about the conflict. Following Tet, peace talks began, and the United States ultimately withdrew from the war in 1973. Following the American departure, Giap remained in command of North Vietnamese forces and directed General Van Tien Dung and the Ho Chi Minh campaign that finally captured the South Vietnamese capital of Saigon in 1975. Death With Vietnam reunified under communist rule, Giap remained minister of defense. After retiring, he authored several military texts, including Peoples Army, Peoples War and Big Victory, Great Task. He died on October 4, 2013, at Central Military Hospital 108 in Hanoi. Legacy Giap was much admired for his military skills, even by leaders in the West, some of whom referred to the Vietnamese general as Red Napoleon. Giap had no formal military training; nevertheless, he was able to drive the Americans to a stalemate in Vietnam and capture the city of Saigon from the South Vietnamese.
